No nights or weekends.
We are looking for a Community Health Worker to join our team.
This is a unique position that is a cross between a direct care worker and a case manager.
This is a full time position offering Health Benefits, Retirement Plan, Flex Scheduling and more.
Duties Include:
Attend in-home participant meetings and other community based appointments.
Conduct comprehensive assessments for non-waiver participants.
Work with family members and other volunteer caregivers to maximize available services to the participant.
Develop a plan of care identifying needed services, supports, and interventions in collaboration with a licensed staff member.
Arrange and coordinate delivery of needed services (utilizing natural supports and community resources).
Monitor participant Person Centered Service Plan to assure delivery of needed services.
Evaluate and modify Person Centered Service Plan as required.
Assist other care managers with questions resulting from assessments and other situations.
Participate in regular staff meeting and training events.
Participate in community events to teach, assist, advocate, and forge community partnerships to manage needs not addressed by natural supports or in-home service providers.
Perform other duties as assigned.
Maintain regular and punctual attendance Requirements:
Minimum of an Associate's degree in a social services field is preferred.
Will consider relevant experience.
Minimum of two years Care Manager Experience preferred.
Experience in the health care system and the field of aging.
Scheduling experience.
Community Health Worker Certification preferred.
Strong computer fundamentals and proficiency with Microsoft Office products.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
